Job Description

Care Services Manager If you love working in healthcare and are confident, caring and enjoy meeting new people, a career as a Care Manager with SweetTree Home Care Services may be just for you. In this role you will carry out assessments of clients needs, put care packages in place to help them remain at home with dignity, independence and control over their life and follow-up the care being delivered to ensure that the desired outcome is being achieved. Success in this role will require not only good community based nursing skills, but also huge passion for making a positive difference in peoples lives. Good communication skills are also essential as the role involves working with clients, their families, their clinical team and other community based care providers to ensure that all of the support required by those we look after is in place.
